Wiltshire Community Respiratory Team - Wilton Health Centre
37.5 Hours per week - Permanent
Wiltshire Health and Care has an exciting opportunity for a Band 4 Assistant Practitioner to join the Wiltshire Community Respiratory Team based in the south of the county.
This position will also be considered as a developmental post for a suitable candidate. This will require commitment to complete the required training.
This role will include working closely with respiratory physiotherapists to deliver a Pulmonary Rehabilitation service (exercise and education programme for people with Chronic Respiratory Disease) across Wiltshire; participate and lead on assessment of patients in preparation for pulmonary rehabilitation programme; support organisation and planning for the day to day running of the service; support data entry for audit purposes and provide clinical and administrative support with the wider team if required. The role will also support the clinical leads and specialist nurses with follow up visits for respiratory visits in their homes and in the clinic setting.
Main duties of the job
This multidisciplinary team delivers a holistic patient centred approach, working collaboratively with Wiltshire Council Active Health programme, Community team services, Psychology service, Community Dieticians, Hospice support services. The team also provides the Wiltshire Home Oxygen Service, Respiratory Specialist Nurse caseload management, medication optimisation and exacerbation management.
The successful candidate will require excellent organisational and interpersonal skills. Good timekeeping, flexibility and initiative are also needed to respond to service needs in this small friendly team. The ability to travel to work in several locations in Wiltshire is required to provide this service therefore access to own car and a full driving licence is essential.
Experience of supervising rehabilitation exercise and knowledge or interest in respiratory conditions will be an advantage, but training will also be provided. There will also be opportunities for respiratory learning and development.
