Calne & Corsham Community Team

30 Hours per Week - Permanent

We have an exciting opportunity for a Band 5 Community Nurse to come and join our Calne, Corsham and Box Community Team at Wiltshire Health and Care. Community Teams within Wiltshire Health & Care provide high quality, complex care in the community setting, which includes caring for some very vulnerable people, often in extremely challenging environments. Wiltshire Health & Care recognises that we need to deliver care closer to home, with the resources available, and be able to meet the needs of the local population. You will be expected to work closely with General Practices, Adult Social Care, local hospitals, and other statutory and non-statutory services to meet the needs of the local population, so excellent communication skills are essential for this role.

Main duties of the job

Experience in front line NHS community patient care is not essential as we are keen to attract new graduates as well as experienced nurses, but evidence of up-to-date clinical professional development is necessary. The role is satisfying and rewarding, and we are looking for enthusiastic,motivated, and hardworking individuals who can work flexibly and efficiently. We are looking for a friendly, caring, motivated nurse to join our active, professional, and dynamic team.

The aim of the role is to care for people in their own home holistically, so they are empowered to control their health and recovery journey. You will also be expected to support patients with long term conditions and end-of-life care.

Calne, Corsham and Box Community team is a busy team caring for patients registered with a Calne, Corsham, Box or Colerne GP, you will have the opportunity to work alongside other members of the multidisciplinary team such as Occupational Therapists, Physiotherapists, Assistant Practitioners, and other Specialist Practitioners. Our Shift patterns are 7-3, 8.30-4.30, 10-6 and 2-10pm and you would work approximately every 4th weekend.

We provide a huge amount of clinical skills training and potential further university courses when you have achieved all the skills necessary for your current role.
