Calne, Corsham & Box Team -Corsham Based

As community services are expanding, there is an exciting opportunity to join the Corsham, Calne and Box Community Team as one of our Community Nurse Specialists.

In this role you will provide support and leadership to a highly skilled team that delivers a catalogue of care and pride themselves on their commitment to the well-being of the community. Ideally the candidate will bring with them clinical experience of working with adults in a community setting and be qualified as a Specialist Practitioner in district nursing. However, if applicants do not already hold this qualification Wiltshire Health and Care offer the opportunity to further train and develop and will support with the studying for and obtaining a Specialist Practice Qualification (SPQ).

We are seeking an enthusiastic and experienced nurse, with a warm personality and great people skills. You will be willing to undertake relevant training, have leadership experience, be able to demonstrate innovative practice and show effective management skills. CCB Team is a busy team across a large area therefore a main factor in the role will be effectively managing the complex patient caseloads spread across the areas, so great organisational and communication skills are essential.

Main duties of the job

If you are passionate about leadership, service development and the delivery of effective, quality care to patients in the community setting then this is the role for you.

You will help lead and ensure high quality evidence-based care is delivered for patients, their families, and carers in the community.

  • Responsible for a complex and diverse caseload, actively working to prevent admission to the acute sector and supporting patients in their preferred place of care.
  • Responsible for maintaining detailed, accurate and confidential patient records, adhering to Trust policies.
  • Leading an effective supportive team, working together to provide clinical advice and support to other staff and patients.
  • Responsible for the management of a defined caseload of patients, with continuing responsibility for the assessment of care needs, the development, implementation, and evaluation of care planning.
  • Accountable for the standards of care and for determining the activity of your team. This will include acting as a role model for staff, sharing learning and developing practice.
  • To support Clinical Lead and Team Lead in service development.
