Rehabilitation Support Worker - Band 3

Amesbury Community Team Amesbury Health Centre

30 Hours per Week - Permanent

Could you support patients when they return home from hospital? Would you like to be a valued member of our forward-thinking community team? If so, then come and join our Home First programme.

We have vacancy for Band 3 Rehabilitation Support Workers, trained to NVQ III standard (or equivalent) or people willing to train to this level. We want to meet applicants keen to develop into this rehabilitation role or who have existing transferable skills.

You will be part of the Amesbury Community Team, based at Amesbury Health Centre, working across a wider area in our South locality.

You will be supported by our physiotherapists, occupational therapists and nurses and will work alongside our Assistant Practitioners and existing Support Workers. You will play a vital role helping people to rehabilitate and regain their independence in their own home. You may be required to support people with long term needs or palliative conditions until alternative care is available.

Main duties of the job

If you have experience in front line NHS community patient care that is beneficial, but it is not essential. We want enthusiastic, motivated and hard-working people who can work flexibly and efficiently. We would like you to have effective communication skills, good IT knowledge, and the enthusiasm to develop.

Wiltshire Health and Care is developing training specific for these roles and will provide you with a supportive, friendly and happy environment in which to learn new skills.

The community teams deliver services from 7am to 10pm, seven days a week. We can talk about your hours of work and working patterns at the interview, or you can call the following manager for a chat.
