Community Physiotherapist - Band 5
Devizes Community Team - Beacon House
30 Hours per Week - Permanent
Devizes Community Team is a busy multi-disciplinary team consisting of Community Nurses, Physiotherapists, Occupational Therapists, and support staff. An opportunity has arisen to work within this professional and supportive team as a Band 5 Physiotherapist.
In this position you will work alongside your Band 6 colleague as part of our supportive therapy team to provide rehabilitation to patients living in the community to improve their quality of life and ensure they remain safe and independent within their own homes. As well as planned interventions you will also provide urgent interventions to prevent un-necessary hospital admissions, including falls, and liaise closely with the local acute hospitals to facilitate timely hospital discharges under the Home First service.
Main duties of the job
The successful applicant will have excellent communication skills, will enjoy working in a team and be able to support junior and support staff. They will be experienced in working with adults with physical health conditions including palliative care.
You will be responsible for line managing junior members of staff and for managing your own caseload within the team.
You will also contribute to managing of the new Urgent Community Response Service alongside your fellow Band 6 Physiotherapist, Band 6 nurses, Occupational Therapists and Assistant Practitioners within the team. You will be supported by Clinical Leads, Advanced Nurse Practitioners and Consultant Practitioners.
