Rehabilitation Support Worker - Band 3
Melksham Community Team
37.5 Hours per week - Permanent
Could you support patients when they return home from hospital? Are you keen to help prevent people being admitted to hospital? Would you like to be a valued member of our forward-thinking community team? If so, then come and join our Home First programme.
We need a Rehab Support Workers trained to NVQ III standard (or equivalent) or a commitment to undertake the training. We want to meet applicants keen to develop into this rehabilitation role or who have existing transferrable skills.
Main duties of the job
Youll be part of the Melksham and Bradford on Avon Community team. You will be supported by our Physiotherapists, Occupational Therapists and Nurses and will work alongside our Assistant Practitioners and existing Support Workers. You will play a vital role helping people to rehabilitate and regain their independence in their own homes.
If you have experience in front line NHS community patient care then thats great but its not essential. We want enthusiastic, motivated and hardworking people who can work flexibly and efficiently. We would like you to have effective communication skills, good IT knowledge, and the enthusiasm to develop.
Wiltshire Health and Care is developing training specific for these roles and will provide you with a supportive, friendly and happy environment in which to learn new skills.
The community teams deliver services from 7am to 10pm, seven days a week. You will work shifts 8-4, 2-10 or 1-9. You will work 2 weekends per month.
