Malmesbury Community Team
30 Hours per week - Permanent
We have an exciting opportunity in the Community Team serving Malmesbury, Royal Wotton Bassett, Cricklade and Purton within the Home First Service. The Home First Service provides therapy, care and support; in peoples own homes for the first few weeks after discharge from hospital, providing support and rehabilitation to promote independence in activities of daily living.
The Home First Service is provided by the Rehab Support Workers within the Community Team with assessment, supervision and support from Occupational Therapists, Physiotherapists and community nurses.
All Wiltshire Health and Care staff are expected to work seamlessly across multi-professional community services and to forge close links with other local services particularly those working in social care. The Malmesbury , Royal Wootton Bassett, Purton and Cricklade Community Team are based in the Malmesbury Primary Care Centre.
The service is provided between the hours of 8am and 9pm, split into shifts of 08.00 16.00, 09.00-17.00 and 13.00 21.00, 7 days a week. This post is for 30 hours per week.
Main duties of the job
The successful candidate will need to be able to provide clear evidence of having previously worked in a caring role, preferably with older people or people with long term conditions. We would either like you to have a NVQ Level 3 or willingness to study to that level. You will also be required to complete your care certificate. Applicants need to have enthusiasm and commitment to working within a community-based team.
