Patient Flow Clinician - Band 5

Fixed Term until March 2024 - 0.60 WTE - 22.5 hours per week

Based at Chippenham Community Hospital

We are recruiting to a 6 month fixed term position of clinician based within the Wiltshire Patient Flow Hub at Chippenham Hospital.

The role involves working to deliver on all aspects of patient flow within Wiltshire Health and Care. This includes taking and processing referrals for onward care, liaising with acute and non-acute hospitals and community based teams. You will need to have a good knowledge of the health and social care criteria and the differing services which are available.

Main duties of the job

You will be a registered clinical practitioner or have a minimum of 1 year experience in a relevant clinical field. You will be able to demonstrate a good understanding of the patient journey and an understanding of how community services support patients.

You will have good communication skills and be able to negotiate with staff from a wide range of backgrounds. You will show attention to detail and excellent record keeping.

The Wiltshire Patient Flow Hub is a small team of staff based at Chippenham Hospital. We work closely with our acute partners, our inpatient wards, social care providers and community teams. We look forward to welcoming new applicants to the team.
