We are
looking for a competent, driven professional to join our increasing First
Contact Physiotherapy (FCP) team as an Advanced Clinical Practice
Physiotherapist in Frailty. This exciting opportunity is to join our existing
Frailty ACP and MSK FCPs in Sarum Trinity PCN, covering Three Chequers Medical
Practice in Salisbury.
Full time
or part time options available.
FCPs are
qualified autonomous clinical practitioners who can assess, diagnose, and
manage problems and undifferentiated conditions, and where appropriate,
discharge a person, without a medical referral. This unique opportunity will
involve working with the GPs, community team clinical leads, Urgent Crisis team
and MSK FCP leads to evolve a new service and to provide high quality,
evidence-based interventions.
You need
to be a self-motivated person, keen to take part in service development, data
collection and evaluation. You must be experienced in working with patients
with frailty and be able to demonstrate the excellent assessment and clinical
reasoning skills that this post will require. Good communication skills with a
variety of people, conditions and within demanding situations will be an
asset.Experience of electronic patient records, multidisciplinary working
and telephone triage will be an advantage.
Main duties of the job
Working
as part of a Primary Care MDT including other ACP and FCP clinicians, GPs,
Nurses, Paramedics, Pharmacists, Social Prescribers and more.
WHC
has a well-established FCP service across Wiltshire (initially set up in 2018)
with clinical leadership from experienced clinicians.
Support
provided to complete the HEE Roadmap to Practice or ACP pathway if appropriate.
Protected
CPD and supervision time, nominated supervisor, Frailty GP mentor and links
with the WHC Frailty Consultant Nurse.
Quarterly
Clinical Forums with opportunities for shared learning, lectures from guest
speakers, journal clubs and action learning sets.
Integration
into the wider Frailty pathway with strong links to community teams and
secondary care.
Funded
training and development opportunities such as MSc level modules, external
courses and Advanced Practice pathways.
Access
to mentorship and career development opportunities to Band 8a level through
completion of competencies.
Flexible
hours and working arrangements available.
Unless
already at proven ACP level, this role is a developmental post and you will
start as a band 7. Once you have worked through the competencies there will be
the opportunity for Advanced Practice competency development to a Band 8a.

- Registered Allied Health Professional (AHP) (Active HCPC registration).
- BSc or equivalent in Physiotherapy.
- Educated to post graduate level or to have equivalent knowledge and experience;
- MSc or equivalent evidence of Advanced Clinical Practice.

- MSc in related field
- Non-medical prescribing
