Community Physiotherapist - Band 6

Permanent - Full Time - 37.5 hours per week

Melksham Community Team - Based at Melksham Hospital

Melksham and Bradford on Avon Community Team is a busy multi-disciplinary team consisting of Community Nurses, Physiotherapists, Occupational Therapists, and support staff. An opportunity has arisen to work within this professional and supportive team as a Band 6 Physiotherapist.

On this position you will provide rehabilitation to patients living in the community in order to improve their quality of life and ensure they remain safe and independent within their own homes. You will provide urgent interventions in order to prevent un-necessary hospital admissions and liaise closely with the local acute hospitals to facilitate timely hospital discharges under the Home First service.

The successful applicant will have excellent communication skills, will enjoy working in a team and be able to support junior and support staff. They will be experienced in working with adults with physical health conditions including palliative care.

You will be responsible for line managing junior members of staff and for managing your own caseload within the team.

You will also contribute to managing of the new Rapid Response Service alongside your fellow Band 6 nurses and Occupational Therapists within the team. You will be supported by Clinical Leads, Advanced Nurse Practitioners and Consultant Practitioners.

Main duties of the job

This position will offer the successful candidate the opportunity of developing both professionally and personally, optimising and enhancing skills and competence whilst working alongside a supportive multi-disciplinary team.

The successful applicant will also be encouraged to contribute to the development and implementation of new ways of working to continually evolve care delivery for our Wiltshire patients and maximise the services within the Community Team.
