Our community Nursing and Therapy teams are currently undergoing a transformation programme as we continue to grow Urgent Crisis Response and NHS@Home in Wiltshire

Our Vision is to integrate our core services, so we have a range of skills to not only support and enable those with long term conditions including Frailty but to also to asses and treat those who are acutely unwell so they can remain in the place they call home.

We are looking for Advanced Practitioners (Nurses, Paramedics, Physiotherapists, and Occupational Therapists) or those who are on a development pathway to come and join our teams. We want people to apply who are confident to assess the deteriorating person and work with a multi-professional team to support recovery at home. This service has a wide variety of caseload where no day is the same and this role will suit those who like to think outside of the box be creative and work closely with our partners in health, care and the voluntary sector.

This is a great opportunity to not only develop expert clinical skills in frailty, respiratory and rehabilitation but also a unique opening to be part of service redesign using new technology and quality improvement methodology to learn, evaluate, adapt and implement.

Main duties of the job

Wiltshire Health and Care is a community provider which is known for investing in staff, supporting career development and agile in embracing change to ensure our services are able to respond to our communitys health needs. We are the bridge between our Acute partners and community health and care and you will be enabled to build system relationships and be part of new enterprises such as neighbourhood initiatives with primary care.

We currently employ Advanced Practitioners across Community services, MSK Out-patients, Heart failure, Respiratory, and In-patients. You will be part of a large peer group and have opportunities to learn together and consider specialising. This is very much a patient facing role and you will be travelling between bases and peoples homes/ care homes. If you have not experienced community care before you, we will give you a full induction.

Working in the community is both rewarding and a privilege; this is where you truly get to understand the many ways that people chose to live their lives and understand the range of health inequalities which sadly exist across our county. Travel time between visits allows you to reflect on your caseload and to work with colleagues and students; teaching and mentoring as you go. Qualifications

Essential

  • Registered with the NMC or HCPC
  • First level nursing or healthcare degree
  • Recognised teaching or mentorship qualification equivalent to level 3
  • Independent Non-Medical Prescriber Qualification
  • MSc in Advanced Clinical Practice or Advanced Nurse Practitioner

Experience

Essential

  • Previous experience in gerontology or a relevant medical speciality at band 6 above.
  • Evidence in challenging clinical practice and leading in the development of new pathways of care and delivering best practice
  • Evidence of successfully resolving complex problems.
  • Experience in carrying out clinical audit and risk assessment
  • Proven skills in leading, managing and developing teams
  • Evidence of successful resource management.

Desirable

  • Experience as an Advanced Nurse Practitioner/Advanced Clinical Practitioner
  • Strong IT Skills - including excel and power-point
  • Knowledge of national NHS drivers and initiatives
