Our team is developing, and a new post has been developed
for a motivated registered nurse in our busy, forward-thinking team. The
successful applicant will help to pioneer improvements in diabetes care in
North Wiltshire.
The community Inpatient Diabetes Specialist Nurse
(CIPDSN) role provides nursing advice for patients, their families, and carers
during their rehabilitation at Savernake, Chippenham, and Warminster hospitals.
helping to enhance their experience, independence,
and recovery.
Main duties of the job
The CIPDSN will facilitate seamless care from acute
and community hospital discharge to follow up in the community. The role will involve face to face and remote
working. As a case holder, the CIPDSN will support patients with acute and
chronic conditions and those undergoing surgery or cancer treatment or enteral
feeding. As a member of the Community DSN team the post holder will exercise a high degree of personal and
professional autonomy, utilising skills, knowledge, and assessment, in the
management of people with diabetes.
The CIPDSN will contribute to service improvement in the community
hospital wards and to community nursing delivery by monitoring and improving
standards of care in accordance with local and national policies. In addition,
the CIPDSN will provide education for ward
based and community teams.
Vehicle owner/ driving qualification essential.
