Wales already has prudent healthcare with co-production being a core principle and brining the two together will be central to delivering sustainable foot services for patients with diabetes. New models and pathways have been developed but implementation at a more local level will be essential to develop a once for Wales approach to reduce variation and ensure equitable and effective care. The post holder will work with the National Strategic Clinical Network (SCN) for Diabetes to develop models of care closer to home, ensuring pathways are implemented to deliver a consistent integrated delivery of care at the point of need. This will also require the post holder to work across other Networks to ensure a whole system approach is met reducing potential forsilo working.
