Job summary

Responsible for the coordination / monitoring of all aspects of the day to day transport provision for Enhanced Care Oncology patients.

We are looking for an individual who is self motivated ,organised who can work as part of a team and can also work unsupervised.

The role will is predominately office based so computer and phone skills is essential

To support the delivery of a customer focused approach to the provision of Oncology transport in Wales through the effective supervision of patient journeys.

Responsible for coordinating patient journey information and planning optimum patient journey and assisting day controllers/planning with the quality assurance of live oncology transport journeys.

Act as a main point of communication between oncology patients, oncology units and transport providers control teams throughout Wales.

Main duties of the job

The Ambulance Care Hospital Coordinator is responsible for supporting delivery of safe, high-quality care that meets patient needs and aligns with commissioner expectations.

The role will play a key part within the Journey Coordination Centre (JCC) and the Enhanced Hubs delivering a customer focused service to patients and health boards ensuring a focused approach to non-emergency healthcare transport.

Key responsibilities include:

  • Acting as the first point of contact for health boards, hospital departments, clinics, patients, and discharge desks.
  • Coordinating patient journey information, with a focus on maintaining quality and performance standards.
  • Developing and maintaining effective relationships to support smooth service delivery.
  • Proactively resolving issues on and off site, with the ability to escalate when necessary.
