Applications are invited for the post of Consultant Renal
Transplant Surgeon at the Wessex Kidney Centre based in Portsmouth Hospitals
University NHS Trust. This is a replacement post to maintain the Consultant
transplant workforce at 6 persons.
The successful candidate will join the current team of five
consultants based in Portsmouth and providing dialysis access and renal
transplant services to the surrounding hospitals. The post-holder will be
expected to have experience of and to participate in the whole range of renal
transplant and access services, both elective and emergency, as well as live
donor surgery. The development of a special interest (to be discussed and not
necessarily in transplantation and renal failure surgery) will be expected as
will interests in education, research and audit.

Main duties of the job
Depending on experience and training, there is an opportunity to be involved in elective and emergency general surgery, paediatric surgery, tertiary vascular access and urology. The Centre provides comprehensive renal services to a population of approximately 2.3 million across seven feeding acute trusts and currently three specialist commissioning groups. It hosts the regional transplant unit which performs 100 to 110 renal transplants per year. The service has approximately 700 haemodialysis patients (including home haemodialysis), 100 peritoneal dialysis (including assisted APD) patients and about 1200 transplant recipients in long-term follow-up. The Centre has 54 in-patient beds across three dedicated renal/transplant wards, its own outpatient department, day-care/assessment areas and home dialysis training facilities. There is a 25-station on-site outpatient haemodialysis facility and 8 satellite dialysis units (3 NHS-run and 5 with commercial partners). Further expansion in both inpatient and dialysis facilities is being planned and imminent.
The job plan attracts ten programmed activities (8 DCC 2
SPA). This includes participation in the renal transplant consultant on call
rota (initially on a 1-in-6 basis) only.

related surgery both in the elective and emergency settings. The clinics are
mainly based on the Queen Alexandra Hospital (QA) site but may be in outlying
hospitals in the future. All surgery is currently performed on the QA site, but
again may be done in outlying hospitals in the future. The surgery will involve
GA and LA lists and emergency on call covering dialysis related problems as
well as renal transplantation and surgical emergencies in the renal patient
population.
The
majority of the post will be as described above. However, there will be
opportunity for individuals to develop a sub speciality role for themselves
depending on their current experience and the requirements of the department.

The
on-call service works with a Surgeon of the Week (SOW) covering from Friday
midday for 7 days including the weekend. The weekday nights are covered by
other members of the team. During this period, all elective commitments are
cross covered by colleagues. The only fixed commitments during this week are
the acute transplant follow up clinics on Monday and Thursday mornings.
There are currently renal surgical clinics on Monday mornings and
Thursday afternoons. There are renal GA lists every Tuesday, Wednesday and
Thursday mornings and a renal LA list alternate Monday afternoons and every
Friday morning. This post holder would be allocated an almost equal share of
the available lists to fit in with on call and elective non-operating
commitments as well as other clinical commitments. The successful candidate
would be expected to work flexibly and provide prospective cover for leave.
Depending on the skills and training, use of other surgical lists and a role in
the emergency general surgical take is available.
The post-holder will be expected to clinically supervise the junior
surgical staff within the department. This currently includes one physicians
associate and 4 junior surgical trainees (who also have commitments on the
general surgery rota). Opportunities for other more focused educational roles
may become available with appropriate training in the future.
The renal and urology services at Portsmouth Hospitals University NHS
Trust (PHU) only provide services for adult patients. Transition of paediatric
renal and transplant patients from Southampton General Hospital happens when
the patient is 18 years of age. With appropriate training, surgical involvement
in these transition clinics may be possible. If the appointment involves
emergency general surgery, then the service at PHT does provide emergency
paediatric cover and emergency operations for children aged 5 to 18. The
transplant team also provide the complex non-renal vascular access service in
parallel to the interventional radiologists and nurse led peripheral vascular
access service.
Portsmouth Hospitals University NHS Trust has increasingly strong links
with the University of Portsmouth as well as the University of Southampton. The
Wessex Kidney Centre is regularly one of the largest recruiters of patients to
portfolio/NIHR studies in the country. The transplant patients that the unit
covers are rarely involved in these studies and therefore offers huge potential
for future research projects. There are opportunities for clinical and
scientific research.
