Are you an experienced, dynamic nurse looking for your next leadership role within a fast-paced and exciting specialist service? If so, Portsmouth Hospitals University NHS Trust (PHUT) wants to hear from you! A very exciting opportunity has arisen for a Clinical Nurse Specialist and Transitional Care Lead within the Paediatric Diabetes Service. This is one of the busiest paediatric diabetes services in the country with over 300 children and young people with type 1 and type 2 diabetes currently receiving active input from the team. The number of patients has increased steadily over the last few years, alongside this the technology used to manage this condition has become more advanced and complex. This role has been specifically created to lead our team of highly committed and experienced Paediatric Diabetes Specialist Nurses and join them in delivering our vision for CYP with diabetes at PHUT. In addition, you will be key in coordinating the transition of CYP from paediatric to adult care.

Main duties of the job

To be the Lead Paediatric Diabetes Clinical Nurse Specialist and Transitional Care Lead at Portsmouth Hospitals University NHS Trust.

In order to act as an effective leader for this specialist service, you will need the skills to act as a coach, mentor, supervisor and role model.

You will be pivotal in driving transformation of care and improving outcomes for children and young people with diabetes within Portsmouth and the surrounding area.

You will require excellent management, communication and interpersonal skills to enable you to build strong working relations with the Adult Diabetes team to align polices and ensure smooth transition for our young people.

You will need to have an interest in contributing and participating in audits and research, enabling local, regional and national improvements, providing and learning from exemplars of best practice across the region.

You will work collaboratively with the services clinical lead to ensure that the services vision reflects the current paediatric diabetes landscape, and to support the team in delivering this vision.

You will be a role model for the other clinical nurse specialists within your team, ensuring that they are supported to develop professionally.

To work with Children & Young People with diabetes to ensure that their voices are heard and that the service is designed with CYP at the heart.

Clinical/Professional

  • Provide professional nursing leadership to the MDT. Managing and reviewing nurse-led diabetes clinics, as well as being an accessible and visible resource across child health.

  • Work with the Practice Education team to review Diabetes knowledge and skills of other nurses and health care professionals within Child Health, and across the wider trust (i.e. Paediatric Emergency Department and young persons transitional care).

  • Ensure 100% attendance by lead Doctor, nurse, and dietitian at PHUT CYP Standards and Quality Committee quarterly meetings and ensure Paediatric Diabetes is part of the agenda.
  • Oversee and manage any new members of staff through the Probation process using the Diabetes Nurse Competencies
  • Facilitate, oversee, and monitor the continuing professional development of the Paediatric Diabetes team by seeking and encouraging and enabling staff to attend relevant training.
  • Contribute to and be involved in national and local audits eg NPDA, spotlight audits. Review audit data to compare our service with others locally and nationally. With the lead Consultant identify areas of shortfall and formulate and implement strategies to address these.
