A very exciting opportunity has arisen for a Clinical Nurse Specialist
and Transitional Care Lead within the Paediatric Diabetes Service. This is one of the busiest paediatric diabetes
services in the country with over 300 children and young people with type 1 and
type 2 diabetes currently receiving active input from the team. The number of patients has increased steadily
over the last few years, alongside this the technology used to manage this condition
has become more advanced and complex. This
role has been specifically created to lead our team of highly committed and experienced
Paediatric Diabetes Specialist Nurses and join them in delivering our vision
for CYP with diabetes at PHUT. In addition, you will be key in coordinating
the transition of CYP from paediatric to adult care.

Main duties of the job
To be the Lead Paediatric Diabetes Clinical Nurse
Specialist and Transitional Care Lead at Portsmouth Hospitals University NHS Trust.
In order to act as an effective leader for this
specialist service, you will need the skills to act as a coach, mentor,
supervisor and role model.
You will be pivotal in driving transformation of
care and improving outcomes for children and young people with diabetes within Portsmouth
and the surrounding area.
You will require excellent management, communication
and interpersonal skills to enable you to build strong working relations with
the Adult Diabetes team to align polices and ensure smooth transition for our
young people.
You will need to have an interest in contributing
and participating in audits and research, enabling local, regional and national
improvements, providing and learning from exemplars of best practice across the
region.
You will work collaboratively with the services
clinical lead to ensure that the services vision reflects the current paediatric
diabetes landscape, and to support the team in delivering this vision.
You will be a role model for the other clinical nurse
specialists within your team, ensuring that they are supported to develop
professionally.

service is designed with CYP at the heart.

management of the Paediatric Diabetes Service along with the Lead Consultant
- To develop and implement strategic plans,

for the assessment, primary and/or secondary diagnosis, management and
evaluation of treatment and care for a caseload of children and young people
aged 0-18 years with diabetes.

Clinical/Professional
