Job summary

Are you an experienced Respiratory Nurse with knowledge and skills relating to Non-invasive ventilation? If so, we have a fantastic opportunity for you. PHUT is looking to expand its complex ventilation service and we are looking for a dynamic, caring and highly professional nurse to become the nursing lead, working with inpatients, outpatients and home care patients.

Main duties of the job

To provide best care for this increasing patient group, we are looking to recruit a committed, enthusiastic and high calibre individual to join our NIV team on a fixed term basis (1 year, with the potential for longer). The team works across a wide variety of areas within the hospital and is led by a Specialist Intensivist, with links to Specialist Physiotherapist, Ward teams and Respiratory Outpatients.

This role would see you becoming an autonomous practitioner, responsible for the assessment, diagnosis, management and evaluation of treatment and/or care for patients attending the Complex Ventilation service. You would develop and manage the service working closely with the Lead Consultant and Physiotherapist for NIV, seeing patients in ED and AMU on admission, through their time on the wards and follow up either in outpatients or in their homes, providing a continuum of care for this vulnerable group.

You will also work alongside other specialist respiratory services including asthma, bronchiectasis, tuberculosis, non-invasive ventilation and sleep medicine and provide a focal point for the training of staff and carers alike regarding NIV.

Clinical/Professional

Manage a caseload of patients including running New patient clinics, NIV set ups, Follow Up Clinics and home visits (planned and emergent).

Help create and then be part of internal hospital NIV Flying Squad responding in a timely fashion to acute admissions requiring NIV input in the Emergency Department/AMU (during core hours); commencing and supporting ventilation as a life support therapy

Develop a service philosophy and models of care to ensure that all policies and procedures are implemented and to lead on the development of nursing practice. Monitor any impact on the patient experience.

Ensure that any patient quality and safety issues are addressed in a timely manner. This includes monitoring and evaluating standards of care by the clinical team and providing regular feedback to the specialty team on the standards of care.

Implement evidence based practice, identify, initiate, manage and evaluate changes that lead to improved and high standards of care and in particular any service changes following feedback from patients. Use information to inform Business Plans for the specialist area.

Communicating, influencing and working with the multidisciplinary team to ensure seamless care across traditional boundaries and barriers leveraging different ways to improve quality and inform clinical decisions. Promote innovation and participate in benchmarking and quality audits to measure effectiveness of current practices.

In collaboration with members of the multidisciplinary team, ensure that the users of the service and their families have a positive experience and using feedback (Elephant Surveys, Family and Friends Test, concerns and complaints) evaluate patient processes and pathways of care are redesigned as required.

Ensure concerns raised by patients and their families are dealt with in an open, honest and transparent way, quickly and effectively, in line with trust policies, procedures and Duty of Candour legislation.

Oversee the service co-ordinator, ensuring that accurate and timely MDT records are maintained, patients are tracked and appointments booked as required, monitoring standards and taking corrective action as required.

Ensure the MDT are competent in the use of any medical devices/equipment and have access to training and monitoring compliance.

Assist in creating/purchasing an electronic patient database and oversee its use by the co-ordinator ensuring accuracy and timeliness of data input.

Make independent clinical, psychological and social assessments of patients and initiate/interpret investigations/treatments.

Reduce emergency admissions by providing timely telephone advice and education to patients and their families. In addition provide education to health care professional both in hospital and in the community.

Responsible for the organisation and management of the service alongside the Lead Consultant and Physiotherapist.

Provide clinical expertise and leadership to relevant staff within and outside PHT, in order to inform service provision and development.

Demonstrate awareness of national and local guidelines relating to specialty (e.g. NICE guidelines) and influence care developments in accordance with these.

Ensure regular team meetings take place and that all members of the MDT have opportunity to be involved in team discussions.

Lead the continuing development of Home Ventilation (Nursing) team, taking responsibility for to Identify, contribute to and lead on aspects in the development and dissemination of evidence-based guidelines, standards and protocols as required and implement change to improve the quality of nursing care.

Contribute to trust peer reviews and any focused CQC style reviews.

Actively contribute to achieving the clinical governance goals/targets set by the trust and department and comply with reporting schedules for monitoring and reporting on clinical outcomes. Includes auditing of services.

Be proactive in the prevention and management of complaints (formal and those via Patient Advice and Liaison
