As a registered nurse working in the community you will provide person centered care based on the holistic nursing assessment in order to plan, implement and evaluate nursing care. You will promote and support self-management and work with people to manage their long term conditions, prevent hospital admission, and support early discharge from the in-patient setting. The role also focuses on using and developing your clinical skills and supporting individuals at End of life.

There are exciting opportunities for you to develop your nursing career and work closely with the wider Adult Health and Social Care team, as well as working with a wide variety of providers, to meet the nursing needs of individuals in their home setting or alternative residential settings.

Experience in community nursing is desirable but not essential as full training will be provided and fully supported.
