A fantastic opportunity has arisen for a Band 7 Community Matron to join our wonderful team of Community Nurses, based in Exmouth/Budleigh/Woodbury.

The post holder will be working alongside our team of Community Matrons within the Community Nurse Teams to support patients with Long Term Conditions and complex health needs. There is an expectation to be a Nurse Prescriber or working towards it and the Specialist Practitioner Qualification for Community Nursing. Are you Confident, kind and committed with an array of skills and knowledge that will enhance and assist in leading the team to continue to promote a strength-based approach, working alongside colleagues in Health and Social Care.

The post holder will be part of an Integrated Health and Social Care Team who are responsible for the assessment, implementation and evaluation of patients needs and the delivery of high-quality care linking in with third party organisations to assist and promote independence. The organisation utilises electronic patient records and therefore an ability to utilise technology is a must. You will continue to develop enhanced clinical skills and to work collaboratively with colleagues to ensure a holistic approach for all patient interactions. You will have the opportunity to utilise creative problem-solving techniques with the support of an experienced team which promotes joint working and shared learning.
