Job summary

To assess, provide and review nursing care, advice and information to patients and carers within the home environment or alternative care setting.

Aim to wherever possible maintain patients in their own preferred place of care, enabling them to maximise their independence and optimise their quality of life.

To work under the direction of the Nurse Specialist (Community) and the Community Nurse Team Manager, using their skills and knowledge as a Registered Nurse.

We are seeking registered nurses to join our Community Nursing Bank offering various shifts to support Community Nursing Teams working throughout the day and night across the towns and rural areas covered by Royal Devon University Healthcare NHS Foundation Trust.

Applicants will need evidence of post registration Community Nursing experience and skill set. They will require good assessment, clinical and communication skills in order to deliver evidence based nursing care to patients within their home environment, recognise when a patient deteriorating and intervene and escalate concerns appropriately to optimise quality of life and reduce the risk of unnecessary hospital attendance.
