Job summary

The Patient Flow Coordinator (PFC) will work closely with the Nurse in Charge (NIC) to ensure a smooth flow of patients in and out of the Acute Medical Unit (AMU) and the Medical Triage Unit (MTU). They will:

Monitor referrals from the Emergency Department (ED) and escalate any patients with long waiting times.
Review the number of referrals from GPs, ED and the ambulance service (SWASFT), anticipate and escalate any issues that would cause delays in receiving patients.
Link with wards to confirm availability of beds noted by site management.
Work with the AMU/MTU nursing team to ensure the transfer and/or discharge of patients occurs in a timely manner.

Support members of the multi-disciplinary team with patient discharge arrangements e.g. booking transport.

An exciting opportunity has arisen for an experienced Patient Flow Co-ordinator to join our team.

The post holder is expected to provide efficient and effective flow coordination, working alongside our Medical, Nursing and Site Management teams to ensure that effective discharge planning and communication are maintained in order to provide a smooth, coordinated journey for the patient from admission to discharge or transfer to a base-ward.

The post holders will be proficient in dealing with confidentiality issues and able to provide excellent customer care service, ensuring patients details are accurately recorded and the relevant documentation is created to support safe patient care.

The Acute Medical Unit is an exciting and stimulating place to work and benefits from excellent team working. This role would suit someone who is organised and practical, and who is proactive and ready for a challenge.

The successful candidate will demonstrate excellent communication skills, the ability to remain calm under pressure and experience of dealing with the general public and with difficult/distressed people.

A flexible approach to work and experience of working in a busy environment and with minimal supervision are also essential.

We are looking for someone who can offer great flexibility aside from their contracted hours, as the post holder will be required to undertake additional hours to cover for colleagues during periods of annual/sick leave.

Working Pattern: Thursday and Friday, 08.15 - 1700

This is a fixed term contract for 12 months. For current NHS employees this post could be a secondment opportunity.
