Job summary

Are you passionate about people and looking to continue your career in Healthcare? We seek a proactive Assistant Practitioner/Trainee Assistant Practitioner to lead upon the management, rehabilitation and discharge of the P2 Rehabilitation Pathway patients within our cluster. This is a new role which will work collaboratively with our Occupational Therapists, Physiotherapists and Rehab Support Workers.

The Community Rehabilitation team based within the Crediton, Moretonhampstead, Okehampton cluster are looking for a proactive and forward thinking Assistant Practitioner/Trainee Assistant Practitioner to join our expanding team. This vacancy has become available due assist with the P2 Rehabilitation Pathway, with this role taking a central role in both the management, rehabilitation and discharge planning for these patients. This post may act as an ideal starting point for a candidate seeking a stepping stone from a Rehab Support Worker role and undertaking appropriate training.

The post holder will work with different medical conditions including stroke, falls, those with long term conditions such as Parkinsons or MS, post-surgery, and those with frailty.

The post is open to people who have already completed their Assistant Practitioner training. For the right candidate who doesn't already hold these qualifications we would appoint the role at a Band 3 and support in obtaining the appropriate qualifications. In addition we actively encourage professional development and have a regular in-house training and a supervision support system in place.

The successful candidate will need to have good IT and communication skills, and have the ability to work independently as well as part of the team.

Working Pattern: 37.5 hours per week, covering a 7 day a week service. This role will include working shifts via a rota across the week so that we support all aspects of daily living. Various shifts are available covering 08:00 - 20:00
