Join Solent to make a difference, supporting the PRRT (Portsmouth Rehab and Reablement Team)!
PRRT is a multi-disciplinary team made from staff employed by Health and Adult Social Care for residents of the Portsmouth City area. The aim is to help patients staff at home and avoid unnecessary admission to Hospital, or to support discharge from Hospital by offering assessment of the patients immediate needs.
You will support the administration for patients journeys from start to finish. By monitoring their journey in Hospital, to ensuring safe discharge from Hospital, and continued support once they are Home, ensuring they have all their needs met.
Working in an enthusiastic, fast paced and at times highly pressured team, you will be working within a multidisciplinary team, and will support the clinical staff with all admin needs to ensure they meet their own requirements of releasing time to care.

Main duties of the job
Word processing of correspondence eg. letters, reports, forms, tables, agendas, minutes & presentation material using appropriate I.T. packages
To send letters & compose routine basic letters within sphere of responsibility.
To use Trust email & intranet systems as required
Answer daily enquiries made over the phone & in person, taking accurate messages. Ensuring messages are prioritised & passed on to the most appropriate person
To ensure that post is collected & distributed in accordance with local team agreement
Liaise with others on matters agreed within sphere of responsibility. On occasions may have to exchange confidential & sensitive information to team members
