Job summary

We can offer flexible hours across a 7-day working week. We are looking for full time but are happy to consider hours from 30 hrs per week upwards.

In this role you will be treating patients in their home, supporting to avoid them being admitted unnecessarily to hospital, or to facilitate their early discharge from hospital. This is a continually developing service and effective leadership is essential. This team also flexes across the Emergency Department (ED) at University Hospital Southampton with the aim to support discharge from ED. This is a fast-paced role where every day is different.

Main duties of the job

  • Professionally and legally responsible and accountable as an autonomous practitioner for all aspects of your workload including management of patients in your care and that of your team
  • Undertake skilled assessment using advanced clinical reasoning and a variety of assessment techniques to provide accurate diagnosis
  • Manage own caseload using specialist knowledge and skills as an autonomous practitioner, including maintenance of comprehensive records
  • Provide highly specialist advice
  • Take lead role alongside the clinical lead and service lead in developing the specialist physiotherapy role
  • Be the lead specialist for the specific clinical area for the Trust using a variety of resources to drive improvement in clinical practice
  • Keep up to date with evidence base, current national and developments within the speciality and utilise knowledge and best practice in actively leading the development of clinical services
  • Maintain a high-level clinical skill set within the team to ensure high quality compassionate care for all patients referred to the service
  • Ensure all care delivered empowers patients to recover independence at the earliest opportunity

Communication

  • To be able to communicate complex, sensitive information to patients/carers where there may be barriers to understanding.
  • To ensure all patients are given the appropriate information & are actively encouraged to engage in their own health needs

Knowledge, Education & Training

  • To maintain own Continuing Professional Development (CPD) and assist the Clinical Lead to assess the CPD needs of the staff.
  • Be responsible for ensuring that relevant training is undertaken by staff in the team and an annual audit of training needs identified as required by the Trust
  • Supervise and assess students on placement, involving close working with the University to ensure academic and practice standards are met
  • Develop and implement induction programmes for new staff
  • Attend regular supervision from Clinical Lead
  • Implement relevant teaching programmes to update staff on clinical care issues
  • Lead on reflective practice discussions within the team and ensure effective outcomes of care are demonstrated.
  • Assist other disciplines within the team to gain their generic competencies in rehabilitation and issuing equipment, signing these off as achieved when appropriate.
  • Lead on equipment provision alongside Band 7 OT, acting as a resource for health and social care staff

Management & Leadership

  • Oversee the supervision of junior team members on a day-to-day basis and ensure that clinical supervision is met within the team.
  • Ensure that students obtain an effective placement in a conductive learning environment.
  • Be responsible for undertaking appraisal and supervision of a defined number of staff, ensuring that mandatory training and other relevant competency training is maintained.
  • Act on behalf of the Clinical Lead in co-ordinating responses to formal/informal complaints and incident investigation including RCA/SIRI preparation and presentation.
  • Manage staff sickness as delegated by Clinical Lead.
  • At all times promote the service within the local health economy and Social Services and to non-statutory organisations to make the best use of resources
  • Lead on specific project areas negotiated with Clinical Lead.
  • Attend relevant meetings in the absence of the Clinical Lead.
  • Ensure self and others comply with Clinical Governance and Health and Safety policies and identify risks to Clinical Lead
  • Proactively and positively engage in service change management to meet the requirement of service delivery.
  • With the Clinical Lead, ensure that the service meets the requirements as outlined in the current service delivery specification including achieving all of its agreed Key Performance Indicators.
  • Staff records are maintained ensuring compliance with the agreed record keeping systems of the Trust.
  • Ensure staff and self work positively with colleagues to maintain effective working relationships and develop team morale

  • Identifies situations of clinical risk & takes appropriate action to ensure a safe environment for patients/clients/families & staff.
  • Contributes to evidence-based practice in speciality areas.
  • Demonstrates commitment to quality improvements, risk management & resource utilisation participating in monitoring & evaluation activities including audit & research activities
  • Provides input into clinical standards/protocols & policies & undertakes clinical audits as required.
  • Evaluates the effectiveness, efficiency & safety of clinical practice contributing to service improvement initiatives.
  • Participates in the implementation of models of care appropriate to patient/client population needs.
