Job responsibilities
Clinical and Professional
*Practice in compliance with relevant code of professional conduct, delivering and participating in the clinical care of patients within scope of practice; being accountable for decisions, actions and omissions at this level of practice.
*Autonomously act and be recognised as a clinical expert in their specialist field using highly specialist clinical knowledge ensuring patient centred care and family involvement, including health promotion and disease prevention
*Apply advanced theoretical and practical knowledge to the clinical care of people, their families and partner agencies involved when delivering and shaping services in line with national guidelines, best practice and the national personalised care agenda.
*Identify and lead the development and implementation of advanced practice within services.
*Hold a caseload that will often be complex and unpredictable: receive referrals, assess, order, interpret and act upon assessments. Consider differential diagnosis, evaluate, treat, plan, refer to primary, secondary and tertiary care or discharge patients, as required.
*Provide advanced clinical decision making based on highly complex facts and situations and encourage critical thinking regarding both ethical and legal issues related to high quality care delivery.
*Develop and maintain expertise in highly developed skills and appropriate competencies ensuring the highest level of precision to ensure patient safety.
*Identifies situations of clinical risk and takes appropriate action to ensure a safe environment for patients/clients/families and staff.
*Identify and implement evidence-based practise, new developments, treatments and technologies into clinical practice.
*Initiate, evaluate and modify a range of interventions which may include postural management programs, equipment, prescribing medicine, care plans, exercise, lifestyle advice, social prescribing, therapies and self-management in accordance with evidence-based practice and national and local policy and protocols.
*Understands and applies the legal safeguarding framework that supports the identification of vulnerable and abused adults/children and is aware of statutory vulnerable patients' health procedures and trust guidance.
*Work independently and autonomously within medico-legal frameworks, making highly complex expert clinical decisions, working within local and national policies being responsible for how these policies are interpreted.
*Be expected to autonomously assess patients, receive and make referrals, assess patients, receive and make referrals, interpret and act upon assessments, consider differential diagnosis, evaluate, treat, plan, refer and discharge patients within the Trust.
Communication
*Produce accurate and complete documentation and patient records consistent with legislation, policies and procedures. These should demonstrate the use of expertise in decision making skill which have informed advanced clinical reasoning.
*Communicate effectively, often about complex, sensitive and potentially distressing information -- such as prognostic indication for poor clinical outcome based on diagnostic markers, or on multi-professional case reviews.
*Anticipate barriers to communication and establish methods of overcoming these, ensuring patients and significant others are kept fully informed and consent to treatment is maintained.
*Written reports will be completed to a high standard to ensure accuracy, relevance and sensitivity, with clear outcomes and recommendations.
Education and Training
*Be responsible for the clinical development of teams in all aspects of clinical practice, facilitating staff to develop specific areas of expertise and proficiency to meet the needs of the caseload within the available resources.
*Participate in the Trust's Leadership development programme and other appropriate developmental opportunities.
*Through supervision and mentorship identify personal learning needs, participating in personal continuing education and other activities to promote one's own personal growth and progress their clinical academic career.
*Lead the supervision and appraisal of Advanced Clinical Practitioners, establishing performance objectives and facilitating individual development opportunities.
*Provide clinical supervision to relevant clinical staff.
*Interpret and evaluate information from diverse sources to make informed judgements about its quality, appropriateness for practice and service delivery.
*Develop and further clinical knowledge and professional skills through relevant training and study.
*Identify and lead the development and implementation of advanced practise within services.
*In conjunction with the Heads of Quality & Professions support services training needs analysis and the CPD application process for appropriate staff and implement new approaches to continual professional development.
*Be responsible for maintaining and disseminating knowledge and expertise within their specialist field.
*Develop and maintain close links with local Universities and participate in the development and delivery of new and established preregistration, post-registration and advanced practice programmes.
*Seize opportunities to apply new knowledge to their own and others practice in structured ways, which are capable of evaluation.
*Work with and empower teams in assessing skills needed for practice, identifying any deficits in this and developing/commissioning programmes to address these issues.
*Work closely with the ACP, Matrons and Head of Quality & Professions and the Learning and Development team to ensure staff have access to clinical updates and achieve required level of competency.
*Support the clinical areas within the area of responsibility to be excellent learning environments for all students to include robust supervision.
*Work closely with the leadership teams and Educator in Practice lead to encourage students to work in the Trust on qualification.
*With their service line leadership team undertake talent mapping and participate in talent management processes.
Leadership and Management
*Uphold and display the Trust HEART values.
*Lead and develop the areas of care in specific care group.
*Provide leadership and be clinically accountable and responsible for trainee ACP's (tACP) and ACP's across the service line and/or within their designated service portfolio.
*Named tACP's and ACP's in the service line will be accountable and responsible to the post holder.
*Foster effective relationships with colleague across a range of specialities and agencies, encouraging productive working.
*Demonstrate compassionate, inclusive, visible and authoritative leadership to Clinical team managers/leads in order to ensure high quality patient centred clinical care is delivered.
*In conjunction with the Clinical Director and Heads of Quality & Professions actively embed the Trust's Clinical Strategy locally within areas of responsibility.
*Develop and sustain appropriate relationships, partnerships and networks to influence and improve health, outcomes and healthcare delivery systems.
*Provide expert advice and support to various strategic groups to develop protocols, clinical guidelines and policies for others to use in practice.
*Identify and lead corporate strategic projects that impact on patient / client care and experience, being responsible for a range of policy implementation.
*Use quality improvement methodology to lead changes in practice and service delivery in accordance with evidence-based standards and policy, challenging traditional boundaries and current thinking; and overcoming resistance and conflict at the Trust, system and national level.
*Work with internal and external stakeholders to develop and deliver service specific pathways and place-based care.
*To support senior healthcare colleagues and clinical governance teams and take responsibility for patient safety, experience and clinical governance issues relating to integrated community services.
*With Operational lead, jointly responsible for initiating, delegating, implementing and monitoring risk management strategies at service level in accordance with Trust policy.
*Promote a culture across the Trust where risk management and governance are known to be the responsibility of everyone.
*Interpret and evaluate information from diverse sources to make informed judgements about its quality, appropriateness for practice and service delivery.
*Maintain a high profile within the Trust providing a strong contribution to the achievement of organisational goals and ambitions.
*Engage and drive Trust strategic ambitions across the integrated care system work programmes, influencing commissioning and provision of outstanding evidence-based practice.
*Provide support and supervision for senior nursing and AHP staff working towards a higher level of practice.
*Participate in the recruitment, selection and retention of staff and manage, develop and support others within the team to support resilience and build confidence in all team members.
*Communicate regionally, nationally and internationally on their areas of specialist knowledge.
*Work in partnership with the existing ACPs, senior nursing, AHP and medical teams, driving service development and change.
*Provide highly specialist expert advice to develop services within the Trust and in association with agencies locally, regionally, nationally and internationally.
*Supports and or leads case review and debriefing activities as required.
