Job summary
We are delighted to offer an opportunity for an Advanced Clinical Practitioner (ACP) or Trainee Advanced Practitioner (tACP) to join our Same Day Emergency Care (SDEC) Older Peoples team based at University Hospital Southampton (UHSFT) emergency care department.
SDEC Older Peoples team boasts a diverse team from both Solent NHS Trust and UHSFT who are supported by medical and nursing staff. Autonomously assessing, investigating, and managing same day emergency medical patients will be the primary role, this will include interpretation of tests and investigations under the direction of a Senior Doctor or Advanced Clinical Practitioner.
The role will involve enhancing the patient experience by administering a holistic approach to patient care, providing leadership and clinical support to medical and nursing staff, acting as a role model for advance practice within the Scope of Professional Practice. This role involves working in a fast-paced environment and being able to work under pressure, respond to changing priorities whilst contributing to key performance indicators.
Supervision and ongoing development will be supported by Consultant Geriatricians, Advanced Clinical Practitioners, and existing Trainee Advanced Practitioners.
Main duties of the job
To develop the freedom to act and make critical judgements to satisfy the expectations and demands of the Advanced Clinical Practitioner role, characterised by highly complex decision making and a high degree of autonomy underpinned by a Master's level award or equivalent, in order to deliver care in an autonomous context.
The post holder will be an experienced registered health or social care practitioner qualified to at least Level 6 Degree Level in relevant speciality area. Individual training needs will be met by undertaking an MSc in Advanced Clinical Practice and a Clinical portfolio, relevant to the speciality area, in order to achieve the level of competence and capability required for the post. They will be expected to gain accreditation with Health Education England's The Centre for Advancing Practice when this becomes available.
The post holder will act as a role model to develop the knowledge to deliver advanced practice skills within their area, promoting patient centred clinical effectiveness in care, developing the skills to work autonomously in delivering independent assessment of undifferentiated and undiagnosed presentations. This will include developing the skills to perform clinical examination, diagnosis, clinical decision making and planning of care and treatment, diagnosis, prescribing, health promotion and safe discharge of patients as required for the role.
