Job summary
Portsmouth Older Person Mental Health team are looking for a motivated, enthusiastic practitioner to join our friendly team, who provide skilled assessments and treatment interventions to the older adult.
The post is within the ICM (Intensive Care Management) Team who support older adults with mental health needs both organic and functional illness who are experiencing a crisis. The service provides specialist support, assessment, formulation and intervention for individuals who are experiencing a significant decline in the quality of life as a result of complex mental health needs/distress that may result in placement/ home breakdown or hospital admission.
To be a successful applicant you will need to be a registered nurse preferably with some experience in care of older persons with mental health. You must be genuinely interested in in working with older adults and provide compassionate care to patients and their families with knowledge of organic and functional illness with a good understanding of physical health problems.
You will be expected to work within the wider MDT implementing and evaluating care, caseload management. You will supervise junior staff and students within the team.
This is a really exciting time in joining our team as the Mental community services as we are going through a transformation, and you will have an opportunity taking part in transforming our service. This service is Monday- Friday 9-5pm however this is likely to change.
A full driving license is required.
Main duties of the job
*To meet the needs of patients and their families to achieve improved health outcomes and promote health and wellbeing while demonstrating care, compassion, competence, communication, courage, and commitment. Care is based on comprehensive assessment, ensures continuity and is patient/client centred, culturally sensitive and evidence-based and which promotes independence.
*Responsible for co-ordinating the work of the team, influencing clinical decisions and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice, underpinned by theory and expertise.
*Manage a caseload/named nurse
*Supervise junior members of staff, students on placements
*To develop and maintain a culture of service improvement which promotes quality and safety and striving to do better.
*This is in line with the requirements set out in the NMC code which require all registered nurses to:
