We are looking for a Senior Mental Health Clinician to take an active role in the wider DDP network in the city, providing support to the Portsmouth DDP network, whilst further embedding DDP informed practice across the team.
We are a Multi-Disciplinary Service providing a range of effective, evidence-based assessments, treatments and support for children and young people (0-25) from all ethnic and cultural backgrounds who experience early developmental trauma, behavioural, emotional and/or mental health difficulties. Types of disorders include Anxiety, Depression, Self-Harm, difficulties forming and maintaining relationships within the context of their trauma, challenging behaviours.
The post holder will be expected to take a lead with management of complex trauma cases and support other CAMHS-STaRT workers with their trauma cases to prevent deterioration in mental health and placement breakdowns. The successful candidate will work closely with colleagues from other services and agencies to promote joint working. To assist in training across sectors and agencies as appropriate.
You'll be responsible for influencing clinical decisions and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice underpinned by theory and experience. (DDP training and progression through the Practicum).
Main duties of the job
- Assess, plan develop/implement/evaluate programmes of care to achieve best health outcome for patient/client/family/health needs as well as actively promote health & wellbeing.
- Evaluate clinical outcome & report variance to clinical pathway, protocols, or best practice guidelines.
- Contribute to effective discharge planning &/or referral process to health care provider & support agencies.
- Apply diagnostic reasoning & professional judgement to practice issues/decisions.
- Manage daily care of patient/client & when required manage care in the absence of senior colleagues.
- Foster inquiry & critical thinking amongst colleagues to advance practice & patient/client care to ensure the provision of positive patient/client outcomes & person-centred care.
- Use the core principles of DDP throughout all elements of practice, alongside other recommended therapies.
- Accurately document assessments of patients/client's health status, diagnosis & response to interventions & treatments using Trust Clinical Records System
- Effectively manage clinical care & area through the implementation of clearly defined policies & procedures.
- Educate & provide information to patients/clients/family/other professionals to improve knowledge of trauma and mental illness, self-management prevention of complications & promotion of recovery.
- Role model safe practice & report poor practice when observed.
- Practice in accordance with legal, ethical, culturally safe, & professional standards including the Code of practice.
