Trainee Advanced Clinical Practitioner in Community Stroke/Neuro Rehab

Job summary

We're looking for a Trainee Advanced Clinical Practitioner in Community Stroke/Neuro Rehab.

This new, exciting post will suit an applicant with a nursing background who is highly experienced in their area of speciality. You will be working towards providing advanced and specialist level comprehensive assessments for patients in the community with highly complex stroke and neurological presentations, case management, treatment and education. Inclusive of, but not limited to, specialist skills within the areas of continence, tissue viability, non-medical prescribing, request and interpret investigations inclusive of blood tests, ECG and ZIO patch as relevant to the needs of the specialist caseload.

You will develop skills to support dysphagia screening and assessment and prevention strategies. This will predominantly be delivered within the setting of their service line in the community, though the scope of practice may extend into primary care, inpatient and clinic settings to support with complex cases and patient pathways.

The post sits within Solent's Neuro Early Supported Discharge Team; a multi-professional team consisting of Physios, OTs, Speech & Language Therapists, Neuropsychologists, and Rehab Assistants who provide intensive specialist neuro rehabilitation to patients within a community setting, usually a patients home, including residential settings, on discharge from hospital or for admission avoidance. We also provides six-month stroke review service to the West Hampshire area.

Main duties of the job

Trainee ACPs are required to attend university teaching days, practise clinical skills in their workplace and complete work-place based assessments of competencies and capabilities related to emerging HEE frameworks. They will also meet their supervisor, manager and University regularly to plan their individual educational needs and monitor their progress across all four pillars of practice.

You are expected to work 60% clinical, 20% University time and 20% work-based learning time with this role as per attached trainee ACP job description and personal specification.

If shortlisted, you will be expected to submit a personal statement in support of your application. This should be an academic, referenced and reflective statement of 1000 words detailing:

1.Why are you planning to undertake the Advanced Clinical Practice MSc program?

2.How will training to work at an advanced level benefit the local patient population?

3.How will training to work at an advanced level change your individual practice?

At the interview stage you will be expected to deliver a pre-prepared presentation for 10min.

This traineeship is to develop staff to work at an advanced level of practice as recognised in the form of an HEE Advanced Clinical Practice digital staff badge. Staff will then be expected to have a further interview/professional conversation to demonstrate their clinical capabilities using their portfolio to be eligible to transition onto the ACP job description.
