The Homeless Healthcare Team is a multi-disciplinary team consisting of GPs, mental health team, nursing and HCA staff, surgery manager and admin. An exciting opportunity has arisen for an experienced, enthusiastic, committed and caring Enhanced Nurse Practitioner to join our busy but friendly and supportive practice to support the clinical team in delivering outstanding patient care.

The successful post holder will be an experienced Nurse Practitioner and Independent Nurse Prescriber. They will ideally have experience of working in primary care and must be confident working autonomously and independently within their professional boundaries and as part of a team whilst carrying out triage, clinical assessment, consultations and prescribing.

Main duties of the job

To act as an enhanced nurse practitioner working independently and in conjunction with other health care professionals

To assess, diagnose and treat the conditions of patients attending within primary care initiating direct referrals as appropriate.

Clinical decision making and monitoring the quality of patient care provided through utilising and maintaining expert clinical knowledge relevant to own field of practice underpinned by theory and experience.

To develop and maintain a culture of service improvement which promotes quality and safety and striving to do better.

Actively contribute to team development on a routine basis
