Job summary
A fantastic opportunity exists for an experienced or newly qualified nurse who has a passion to deliver the best care for our patients on our wards. A re-structure within Adults Southampton creates a vacancy for a B5 Registered Nurse supporting our Inpatient Rehabilitation wards at an exciting time for rehabilitation within Southampton, South-West Hampshire and nationally.
Working alongside the Ward Manager and Allied Health Professions, Service Manager, and Clinical Leads as part of the wider service team, responsible for delivering improved health outcomes and wellbeing for our patients and their families.
Patient safety is paramount therefore knowledge of the Nursing Process, healthcare governance, health and safety and infections prevention is required.
We are looking for a motivated and enthusiastic individual with a passion for rehabilitation and excellent outcomes.
We currently operate two level 3 rehabilitation units, a Level 2b Neurological Rehabilitation unit and a Neuropsychiatric unit which provide a range of specialist rehabilitation and as part of Solent's, exciting 'Futures project' we are moving to bring all our community rehabilitation units onto a single site with a new purpose built 'state of the art' rehabilitation centre.
The two rehabilitation wards currently based at the Royal South Hants Hospital are;
Fanshawe Ward is a 19 bedded ward and Lower Brambles is a 24 bedded ward. Both provide beds supporting acute admission avoidance and rehabilitation.
Main duties of the job
*Proactively and positively contribute to the achievement of clinical outcomes through individual and team effort.
*Ensure that resources are used effectively and in accordance with agreed procedures
*Build and sustain effective communications with staff and other healthcare professionals involved in the shared services as required to ensure good team working and collaborative working practices
*Maintain and continuously improve in depth specialist knowledge relevant to the clinical area which significantly contributes to the Trust's stated objectives and aims.
*Undertake evidence-based practice for either developments relating to service opportunities or patient involvement around health issues and contribute towards the continuing development of professional nursing practice.
*Dissemination of knowledge through report writing, and reviewing, taking full responsibility for clinical accuracy and reliability and being sensitive to the wider implications of that dissemination.
*Be responsible for the assessment of patients' care needs and developing and implementing programmes of care/rehabilitation, for patient with complex medical conditions including prioritising within a range of care/treatment options. This includes:
-Tissue viability
-Dysphagia
-Nutritional risk
-Continence assessment
-Vital signs
-Discharge planning
-Moving and handling
