Job summary

Are you passionate about providing patient centred care to people with Neurological conditions? We're recruiting a Physiotherapist to work within the Community Neurology Service (CNS), at SMCHC and the Neuro Gym Outpatients department at QA.

We're looking for a skilled physio who is committed to working in a multi-disciplinary team. We're focused on delivering neurological assessment and treatment plans centred around patients goals to improve independence and quality of life.

The CNS treat patients with a wide variety of neurological conditions in the setting that best meets their needs. Patients will have diverse and complex neurological conditions such as Parkinson's disease, MS, MND and Traumatic brain injury as well as other neurological conditions such as stroke, cerebral palsy etc.

The outpatient team provide individual and group treatment which is based upon jointly agreed realistic and achievable goals between the patient and therapist.

This is a rewarding and challenging role caring for patients with long term complex neurological conditions and providing opportunity to work within a vibrant team with patient care at our core. You will have the opportunity to be involved in service and quality improvement and develop leadership skills as well as supervising rotational physios, therapy assistants and students on placement.

Main duties of the job

Work flexibly between the 2 locations to provide skilled physiotherapy assessment, clinical reasoning and treatment

Be accountable for all aspects of your own work, including the management of patients in your care.

Undertake a comprehensive assessment of patients including those with diverse or complex presentations and co-existing pathologies; use advanced clinical reasoning skills and manual assessment techniques to provide an accurate diagnosis of their condition.

Formulate and deliver an individual physiotherapy treatment programme based on a sound knowledge of evidence-based practice and treatment options using clinical assessment, reasoning skills, and knowledge of treatment skills e.g. manual physiotherapy techniques and handling skills, patient and carer education, exercise classes, electrotherapy techniques, and other alternative options.

Identify health and social care problems requiring assessment /intervention from other agencies and with the consent of the patient action those referrals.

Take delegated responsibility from the physiotherapy clinical lead and community neuro service team leader for managing patients and be responsible for providing specialist physiotherapy assessment and treatment plans.

Formulate accurate prognoses and recommend the best course of intervention, developing comprehensive care plans in conjunction with the patient, carer, and the multi-professional team.

You must have access to a vehicle for work and a full driving license is essential.

To provide and receive complex and sensitive information to patients, relatives, and colleagues with empathy, persuasion, and reassurance, and ensure information is clearly understood, where there may be barriers to understanding.

Communicate effectively with members of the multi-professional team or to whom the patient's care is transferred to ensure a consistent multidisciplinary approach to care.

To ensure timely and effective communication with the service clinical leads, team leaders, and other senior staff on all professional matters.
