Job summary
The PCAT team is at the centre of making sure that the next stop for Portsmouth patients leaving hospital but who still require care, have the best Community option open to them.
We have a role for you that will see you at the heart of Acute and Community decision-making around individual patient journeys. It is a senior role that will require corresponding skills of in-depth, complex patient assessment and triage, as well as supervision of other colleagues.
As a qualified nurse based primarily at Queen Alexandra Hospital, you will use your expert, holistic appraisal skills to determine the best next steps for Portsmouth patients as they move out of an acute setting and into the Community.
Liaising with patients, their families, as well as Health and Social Care colleagues and peers, you will find your proven aptitude for listening and for communication, as well as for backing up your decisions with expert, professional analysis and skill, come to the fore.
This is a senior nursing role, which we are happy to offer out as an opportunity for existing senior nurses, but also as an acting-up, secondment opportunity to an appropriate, qualified candidate.

Main duties of the job
Applies diagnostic reasoning and professional judgement to nursing practice issues/decisions.
Manage care of patients/clients on a day-to-day basis and when required manage care of all patients/clients in the absence of senior colleagues.
Fosters inquiry and critical thinking amongst colleagues to advance nursing practice and patient/client care to ensure the provision of positive patient/client outcomes and person-centred care.
Identifies when a child may be at risk and knows how to refer to the local multi- agency safeguarding hub (MASH) when appropriate.
Identifies when a vulnerable adult may be at risk and knows how to refer to the local authority adult safeguarding service.
Provide opportunities for junior colleagues to assess their work and to provide them with clear, sensitive, and appropriate feedback that will help them improve and develop.
Maintains and develops own clinical expertise and knowledge in speciality evidence-based nursing practice and personal portfolio to demonstrate fitness to practice in line with expectations for revalidation.
