Job summary

Are you a nurse leader looking for a new challenge? We are looking for a Matron / Clinical Team Lead to join our team!

As the Lead nurse for Diabetes, you will provide and manage the Diabetes specialist nursing team for Portsmouth City offering specialist knowledge, support, advice and education in relation specifically to Diabetes and Diabetes nursing. You will work primarily with health care professionals but also to those living with Diabetes and their carers', where appropriate.

You will be responsible for the nursing staff within the Diabetes specialist nursing team including recruitment, appraisal, continuing professional development and human resources.

As team lead you will ensure collaborative working across the Portsmouth Localities to promote, deliver and support the continued development of the Portsmouth City Diabetes service model within the service specification, you will provide and shape a service which enables general practice to improve outcomes for people with Diabetes within Portsmouth.

Main duties of the job

Provide professional leadership to nursing & associated professionals across the service

Work closely with the Matrons/Head of Quality & Professions & Service Managers to ensure the operational delivery of high-quality care, clinical safety & effective clinical governance

Ensure that care is consistently delivered to a high standard & that the nursing & professions resources is effectively managed in all inpatient areas

Be a highly visible, accessible & authoritative presence providing leadership, advice & support

Be accountable/work to ensure the delivery of safe, caring, effective, well led clinical & multi professional care

Deliver effective skilled clinical leadership & pro-active resource management including the delivery of all aspects outlined in the Matrons Handbook (2020)

Provide/manage the specialist nursing team for Portsmouth, offering specialist knowledge, support/advice & education to health care professionals & those living with diabetes & their carers

Manages & is responsible for the nursing & admin staff within the diabetes specialist nursing team including recruitment, appraisal, continuing professional development & human resources

Provide & shape a service to enable Primary Care to improve outcomes for people with diabetes

Integrate work streams & pathways with Community Nursing Primary Care networks, providers/ stakeholders relevant to service

Ensure efficient use of resources by enhancing cross working, where possible, with other community specialist services
