Job summary

We are looking to recruit to an exciting position in Adult Mental Health to be based at St Marys Community Health Campus in Portsmouth!

We are looking for Mental Health Practitioners to join our responsive Community Mental Health Recovery Team, whether you are a registered Mental Health Nurse -- we want to hear from you!

Our team is an established integrated multi-disciplinary team; we pride ourselves on being innovative, creative, and responsive to meet the needs of our service users.

Working in Portsmouth offers a diverse inner-city client base where we provide intensive treatment and interventions to individuals who present with a range of mental health needs. We need you to travel across the city to visit our patients, either by bike or bus, car or train! We also have pool cars which can be used by people with full UK driving licences and if you want to use your own, you can claim mileage!

Main duties of the job

Our aim is to prevent hospital admission and to be the access point for urgent mental health care to adults across Portsmouth. We work alongside our neighbouring teams to ensure patients who are in crisis are offered practical alternatives to hospital admission by the right professional at the right time.

We're passionate about providing you with the opportunity to develop and have a wide range of therapies and treatments to train in to continue your learning. This could include DBT, CBT, prescriber or CAMS training. We want the best for you so you can deliver the best quality care to our patients.

Although this is a community based role, our offices are based at St Marys Community Health Campus in Milton. This offers you a space to meet and connect with your team, time to complete outstanding work and an affordable restaurant to give you the fuel to keep going all day long! We also have hours and working patterns to suit you and your home life.

Job description

Job responsibilities

*To ensure the maintenance of a high standard of care to working age adults with severe and enduring mental health difficulties supporting the implementation of the Care Programme Approach (CPA) and act as care coordinator to an agreed number of cases.

*To offer comprehensive needs assessment and care plan, Risk assessment, Dialog assessment ensuring the implantation and review of appropriate care and treatment meeting the needs of Service Users in an effective, efficient and recovery focussed approach.

*To maintain a commitment to service user and carer empowerment and to assist and encourage the development of initiatives which facilitate their involvement in their care and at wider service planning.

*Develop, support and mentor new staff including students, trainees, and support staff.

*Organise and arrange, chair, CPA meetings, Professional Meetings, MDT, Case Conferences, and other clinical meetings as indicated.

*Provide comprehensive reports for Review Tribunals, Managers Hearings and Social Care Reports and present the information to a good standard in writing and at meetings in a professional and positive manner.

*To be able work as a team member in a flexible facilitative manner.
