Job summary

The purpose of this role is to assist in the provision of care and treatment for the young people who have presented in acute mental health crisis at QA hospital.

This is 12 month fixed term role to cover secondment.

Flexible shift pattern over 7 days, 9am-10pm Mon-Fri and 9am-5pm weekends and bank holidays

This includes:

- Working as an autonomous and accountable practitioner within an integrated team, supported by the senior practitioners

- Assessing children and young people who are in crisis, formulating risk management plans and making referrals to partner agencies and services

- Providing specialist mental health advice and input within discharge planning meetings

- Offering post-assessment reviews, psycho-social interventions and self-harm minimisation tools, to effectively manage risk and support a young person's safety within the community.

The role involves shift work, over a 7 day period. Covering hours of 9am-10pm Monday to Friday and 9am-5pm weekends and bank holidays.

Flexible working pattern available on request.

Main duties of the job

  • Undertaking rapid crisis assessments for young people referred through the general hospital and, providing short crisis intervention and signpost to other specialist services as required
  • Contributes to effective discharge planning and referral processes to health care providers and support agencies to meet identified health needs.
  • Based on assessment, to formulate & convey sensitive, complex, & sometimes unwelcome information to clients & other professionals and manage this as constructively as possible.
  • To support senior practitioners completing complex assessments, though the gathering of information, completing referrals and liaising with partner agencies
  • To work alongside senior practitioners, in providing and sign-positing to specialist advice, specifically relating to child and adolescent mental health, to professionals in other agencies/organisations working in the field of child and adolescent mental health
  • Accurately documents assessments of patients/client's health status, diagnosis & response to nursing & other interventions and treatments on the Trust Clinical Records System
  • Identifies when a child may be at risk and will refer to the local multi- agency safeguarding hub when appropriate
  • Practices in accordance with legal, ethical, culturally safe, & professional standards including the Code of practice

The CAMHS liaison team provide:

  • Assessment of children and young people who have presented at QA Hospital with mental health concerns such as self-harm, overdose & suicidal ideation
  • Liaise & build positive relationships with hospital staff & wider network services such as social care and schools
  • Co-ordinate safe discharge and follow up plan from QA Hospital
