Job summary
The post holder will be responsible for supporting patients and their families through the optimal breast cancer pathway across secondary, community and primary care as required by the patient. They will work under the indirect supervision of the breast cancer nursing team and be responsible for coordinating care for people with complex care needs and providing a point of access to patients and carers. The successful candidate will work closely with the breast multidisciplinary team (MDT) which includes surgeons, breast care nurses (BCN's), radiology team, cancer services, operational and administrative colleagues.
The post holder will contribute to the practice development within their clinical care setting and work in accordance with Trust Policies, Procedures and Guidelines and will be responsible for completing any work-based training associated with the role.
Main duties of the job
The post holder will deliver an essential role within the breast service through developing and utilising effective tracking processes to ensure that cancer patients receive timely appointments and treatments in line with their clinical management plan. The tracking of cancer patients ensures that all patients are accounted for at any point in their pathway and that processes are in place to ensure robust monitoring of all patients.
The development of the role will be led by the needs of the patient; the successful candidate therefore will be adaptable and willing to work flexibly. As the service progresses the post holder will be supported to work autonomously and to increasingly exhibit their initiative towards hastening patient's pathway to treatment.
The post holder will practice autonomously and be responsible and accountable for safe, compassionate, person centre care that respects and maintains dignity and human rights.
The post holder will practice in a holistic, non-judgmental, caring and compassionate manner that avoids assumptions, supports social inclusion and recognises and respects individual choice and acknowledges diversity. They will work in partnership with other health and social care professionals, students and trainees and agencies, service users, their carers and families.
