Band 4 Cancer Pathway Coordinator

Job summary

An exciting position has become available within the cancer services department for a Cancer Pathway Coordinator. This role will be specific in preparing and attending an MDT meeting, taking live minutes, tracking cancer patients through their pathway, liaising with internal and external stakeholders in ensuring that data is processed in a timely and effective way. Candidates will need to be assertive, confident, flexible and able to work under pressure. Attention to detail and a keen eye for data quality is essential for this role.

Cancer Pathway Coordinators assist in the achievement of cancer targets by capturing patient waiting time information, monitoring cancer waiting times by proactively identifying risks and take corrective actions by informing General Managers of potential delays to ensure the cancer targets are not breached.

You will also support and attend the tumour site patient tracking meetings through the preparation of patient tracking lists, capturing of actions and following up progress via agreed escalation protocols.

The post holder will be required to provide cross cover to MDT meetings which may require you to travel across sites.

Main duties of the job

Cancer Waiting Times

*Collaborative working with clinical teams both within/outside of the trust to ensure appropriate management of patient information and co-ordination of patient pathways. Work with trust staff to ensure all cancer patients have all necessary appointments, investigations and procedures arranged in a timely manner and record details of patients coming via all referral routes. Establish robust processes with all involved in cancer patient pathways to negotiate and improve the co-ordination of the patient journey e.g. X-ray department.

*Assist in the achievement of cancer targets by capturing patient waiting time information, monitoring cancer waiting times by proactively identifying risks and take corrective actions by informing General Managers of potential delays to ensure the cancer targets are not breached.

MDT Support

*Prepare material for specific hospital sites when the MDT is being held on a different hospital site both within the Trust and within the regional centres and networks. Working with the individual teams, ensure that there is a system to ensure communication is maintained and all material sent is returned.

*To attend and minute the weekly MDT meetings including typing and displaying information live during the meeting in order to ensure accurate recording of recommendations are agreed and signed off by the clinical team

*Distribute updated patient outcome information in a timely manner, for patients referred from both internal and external organisations
