An exciting opportunity has arisen to join our friendly Upper GI Cancer Clinical Nurse Specialist Team. We are seeking applicants for the post of Personalised Cancer Support Worker. The successful candidate will work alongside the Cancer Clinical Nurse Specialists in supporting the assessment of patients' holistic needs and helping patients and their families navigate the complex health and social care system. We are a very quick thinking very supportive Team who value our patient care, support and each other. Any individual who feels they can join our team and have a basic knowledge of Cancer and Speciality pathways. A working knowledge of UGI and Sarcoma pathways would be an advantage

Main duties of the job

Main Duties

This is an exciting opportunity for the right candidate to join team to carry out a varied range of tasks to assist patients on their cancer pathway including managing phone calls, providing patients with information and general coordination of patient's pathways

You will be skilled in assessing and meeting individual patient information needs, whilst facilitating access for all patients to the Holistic Needs Assessment and Care Plan being sensitive to assessing requirement for e.g. benefits advice, family support and advocacy support.

We seek a calm, dynamic, motivated and enthusiastic person who possess excellent communication, assessment and problem solving and IT skills. You will have a background in health or social care, evidence of being a team player whilst having the ability to work on your own within the boundaries of the job description. Previous experience working clinically with patients with cancer or other types of chronic life changing conditions is essential.

Comprehensive induction and on-going education and training will be provided.
