The successful candidate will have a minimum 2 years of experience and knowledge in renal nursing and dialysis.

Main duties of the job
As Band 6 Advanced Kidney Care Nurse Specialist you will assist the lead nurse in the management of a designated caseload. This will include nurse led clinics across 3 sites and the delivery of virtual and face to face education sessions to support patients with chronic kidney disease throughout their journey. Knowledge of different renal replacement therapies is essential to assist patients to make a modality plan as function declines.
You will have excellent communication skills and deliver a smooth transition onto peritoneal dialysis, haemodialysis or supportive care in collaboration with the lead renal consultant and multidisciplinary team to ensure a quality and effective service.

A clean drivers licence and access to a car is essential for visits in the community
