Job summary

We are excited to offer an excellent opportunity for an experienced and enthusiastic HF CNS to join our heart failure service as a Band 7 HF CNS. This post offers the chance to be part of a proactive, multidisciplinary team that delivers high-quality, patient-centred care across both hospital and outpatient settings. You will be based at Heartlands, Good Hope, and Solihull Hospitals and play a key role in the assessment, management, and ongoing support of adults living with heart failure.

As an HF CNS, the core aspect of your role will be patient education, prescribing heart failure treatments, creating treatment plans, and arranging follow-up after both inpatient and outpatient reviews, in collaboration with cardiologists, GPs, and wider primary care and community teams.

We are looking for a Registered Nurse with post-registration experience in managing heart failure, who is an NMP and holds a health assessment qualification. You should have, or be working towards, a relevant specialist qualification such as an MSc module in heart failure. Strong clinical decision-making and communication skills, as well as the ability to work both independently and as part of a wider MDT, are essential.

Main duties of the job

In partnership with members of the multidisciplinary team the post holder will develop, implement and evaluate seamless specialist service. They will ensure patients receive the highest standard of clinical care. As a nursing expert the post holder will demonstrate a high level of expertise within the specialist's service, providing advice, education and support to staff, patients their families and carers.

The post holder will lead on clinical audit activity within their specialist area. The post holder is responsible for contributing / leading the on-going development of clinical practice, standards of care within the service including the development of policies, procedures, protocols and guidelines. They will undertake service developments in collaboration with multidisciplinary colleagues. The post holder is accountable for their safe, effective clinical practice and working as part of a defined team and supporting team performance, ensuring efficient and effective use of physical and human resources. They will provide a range of educational and training packages for health care staff and patients.
