Job summary
This is a fantastic opportunity to join the energetic, innovative Complex Discharge Team at Good Hope Hospital. It is an exciting time for Complex Discharge with many new pathways and initiatives being introduced and partnerships with external agencies being forged by the complex team.You will provide clinical expertise to support safe, effective and timely discharge working directly with internal and external multidisciplinary teams as required.
This post will offer you the opportunity to develop and flourish whilst learning about the operational management of the Trust. You will be part of a developing team that strives to ensure all patients are given equal opportunity to return home.
You'll need to be enthusiastic and have a strong sense of purpose and good diplomatic skills in engaging staff. However you'll also need to be tenacious, confident, calm and persuasive.

Main duties of the job
Act as key-worker to co-ordinate individual complex discharges
Act as Trust experts in discharge planning, assisting and supporting the MDT to drive and co-ordinate discharges, ensuring effective, timely intervention from all disciplines
Assist wards and teams with understanding and using the 'Discharge to Assess' model
Complete Health Needs Assessments to facilitate discharge
Complete Fast Track Assessments for palliative patients
Provide education in discharge planning to members of the MDT or other professional groups requiring this teaching
Support transfers to NHS funded community venues/services
Monitor performance associated with discharge, identifying gaps in service provision that may have a detrimental effect on length of stay, delayed transfers of care or patient experience
Work with multi-agency representatives to reduce length of stay and delayed transfers of care, to achieve performance targets related to discharge
Essential skills
Evidence of knowledge of the Hospital Discharge and Community policy
Evidence of knowledge of Discharge to Assess and the hospital discharge pathways.
Ability to demonstrate the importance of, and commitment to integrated working

Proven post registration experience at ward level
Success in working collaboratively across professions and services
