Job summary
The post holder will provide a comprehensive and full ranging administration/co-ordination service, working in conjunction with colleagues, service users and stakeholders to provide an efficient service.
The post holder will play a pivotal role in both tracking patients through their clinical pathway ensuring the waiting times targets/ admission and discharge or transfer of care processes and pathways are being met, recording relevant data along the patient's journey.
With a focus on urgent and emergency access and inpatient stay the post holder will work closely with the clinical and operational management teams ensuring all patients referred to and cared for by the speciality are tracked and have treatment and management plans and that there are effective discharge or transfer plans in place. They will maintain accurate and comprehensive records of patient information which relates to admissions and discharges (planned and unplanned, including intra hospital transfers and urgent referrals and internal referrals).
They will attend handover / meetings and obtain information to enable clinical / operational teams to make informed decisions about patient flow relating to planned and unplanned referrals and discharges.
The post holder will exercise initiative and judgement using acquired skills and knowledge, maintaining confidentiality at all times. The post holder will provide an empathic and sensitive point of contact for service user.
Main duties of the job
1.Collaboratively communicating and working with clinical/operational teams both within and outside of the trust to ensure appropriate management of patient information and co-ordination of patient pathways.
2.Record / coordinate and communicate details of patients being: admitted, discharged, transferred in /out of department.
3.Has awareness of the following at ward /service level :
-IPC status of patient
-Sideroom use
-Patient that need close monitoring
-EOL care needs
-Potential/actual discharges
-Vulnerable patient
4.Establish robust processes with all involved in the service patient pathways to negotiate and improve the co-ordination of the patient journey e.g. X-ray department/ discharge hub etc.
5.Support the nurse in charge of the service/ ward / unit to monitor and ensure the most effective use of beds within the service.
6.Monitor appropriate admissions and in patients progress on a daily basis and prevent discharge delays wherever possible.
7.Escalate any delays in patient flow to clinical and operational teams.
8.Liaise with the clinical site teams / outpatient departments and other hospitals/ services to keep them informed of decisions to admit / discharge patients,
9.Have responsibility for the timely and accurate data entry of patient information and use the system to reports patient information.
10.Exercise initiative and judgement based on acquired knowledge and experience of service/ speciality.
