Job summary
This is a new exciting role as part of our growing intestinal failure pharmacy team.
you woul be working mainly at the Queen Elizabeth Hospital site with a once weekly commitment to Heartlands Hospital or Good Hope to establish further Nutrition Support Team ward rounds.
1) To partake in specialised clinical pharmacy duties ensuring core clinical pharmacy activities are undertaken (60%)
2) To support and develop the Adult Parenteral Nutrition and Intestinal Failure clinical pharmacy service provision for the nutrition support team (NST) working with other pharmacy staff providing safe, effective and economic use of medicines.
3) Leadership, management and directorate/service support roles (20%)
4) To line manage and support more junior pharmacists in the listed directorates (10%)
5) To take full part in clinical/dispensary rota and bank holidays duties (10%)
We would be open to a step approach and to delevloping a 7 into the 8 post.
Main duties of the job
**To provide specialist input and demonstrate expert knowledge in Adult Parenteral Nutrition and Intestinal Failure in accordance with the priorities set by the Chief Pharmacist and the Trust agreed standards. Working within the trust formulary, using evidence based medicine, across UHB sites.
*Ensure the clinical services provided are of a high standard and that all prescribed medication is clinically screened in a timely manner. Provide daily clinical pharmacy services to Adult Parenteral Nutrition and Intestinal Failure wards.
*Develop the clinical pharmacy service and lead other Pharmacists and Technicians in Adult Parenteral Nutrition and Intestinal Failure service to a high standard of pharmaceutical care.
*Provide highly specialist advice to and counsel patients and/or their relatives (who may sometimes be distressed and have language difficulties or other disabilities) on their prescribed medication to promote compliance.
*Monitor and report discrepancies found in the computerised prescribing system (PICS), liaising with the Lead Pharmacist- Electronic Prescribing.
*Develop a whole systems approach within the specialty and develop integrated care pathways to provide information to patients on obtaining supplies of specialised medications after discharge from hospital.
*Participate in the rota for provision of pharmacy services covering dispensary, on Saturdays, Sundays and Bank Holidays. Providing advisory support to the on-call service as per rota.
