Job summary
A new and exciting opportunity has risen to join our supportive and friendly team here on Wellcome Short Stay Unit as Patient Flow Coordinator. There will be opportunities to help contribute to effective teamwork, exchanging your views and ideas to continue to develop the best practice. There will be many training and development opportunities throughout your time here within the Trust.
We are looking for someone who works well within a team and independently. Someone with great organisational and communication skills with both their team and service users, in person, electronically and over the telephone. The Flow Coordinator will have good written and communicative English skills.
The post holder will be required to work full time hours, on an agreed shift pattern over a 7 day working week.
Main duties of the job
The post holder will assist the flow throughout the unit on a daily basis, keeping up to date records on patients being treated and managing admissions, discharge and transfers in a timely manner. The Flow Coordinator will be expected to have effective communication when liaising with multiple disciplinary roles throughout the hospital. As well as displaying empathetic and a sensitive point of contact for the service user.
The Flow Coordinator will attend handover/meetings and obtain information to enable clinical/operational teams to make informed decisions about patient flow relating to planned and unplanned referrals and discharges. The post holder will always maintain confidentiality, be able to use initiative and judgement as well as the required skills and knowledge.
