Job summary
Are you an Advanced Clinical Practitioner or wish to develop the necessary skills as a Trainee ACP with a passion for community care? UHB Community have an exciting new opportunity for you to join our evolving team, providing quality, compassionate care to patients in their own homes.
We are embarking on a transformation programme which includes developing new roles to lead, develop and deliver person-centred, outcome-based care as part of the Urgent Community Response and Virtual Ward programmes. You will play a critical role in supporting patients as part of the strategy across Solihull to ensure care is provided by the right person, at the right time, in the right place, ensuring advanced assessment and care planning which enable patients to remain and be managed safely at home.
We are looking for people who can provide outstanding clinical care whilst being able to lead and inspire others with experience of engaging across multiple health and social care partners. You will be able to support others, encourage their ideas and perspectives, and prioritise effectively to support the service delivery.
For trainee applicants you must be willing and able to undertake and complete MSc in Advanced Clinical Practice
Main duties of the job
In this role you will actively support the development of admission avoidance pathways within an evolving integrated service to deliver safe and effective care and admission avoidance pathways across the local health, social care and 3rd sector economy.
You will be responsible for delivering and supporting high quality care and act as an expert practitioner and a role model for the team demonstrating advanced clinical practice skills and decision making offering a patient centred approach to care planning and maximising independence and well-being of those referred to the service.
You will assist with the development of an integrated approach to the delivery of service involving all relevant stakeholders including health and social care organisations to achieved reduction in admission and embed robust admission avoidance pathways in the community.
You will be able to manage your own workload and work unsupervised supported by appropriate and regular clinical supervision.
You will act as a clinical expert in direct contact care, able to make high level skills decisions, informed by advanced diagnostic, assessment and treatment skills.
You will be able to drive and have access to a car to use as part of your working day
