Job summary

A fantastic opportunity for experienced Occupational Therapists in Solihull Community Services Intermediate Care Bedded Units Team based at sites across Solihull is expanding providing therapy to more people who are placed in discharge to assess beds requiring further assessment and rehabilitation following a period of hospital admission and have complex discharge planning needs.

The trust and team are committed to providing a 7 day therapy service and participation in a weekend/ bank Holiday rota. This would suit an experienced Band 5 who is ready for career development or a current Band 6 who wishes to work in a team setting to provide ongoing assessment and treatment progression of complex discharges from the acute hospitals.

Main duties of the job

Deliver a high standard of therapy provision in various community settings within the Home First /Discharge to assess model.

Provide assessment and rehabilitation to our patients; promoting recovery, reducing risk of admission and facilitating discharges from the acute setting into the Community.

Manage complex discharge planning needs by working collaboratively with in an interdisciplinary therapy team and wider multidisciplinary team. This required effective communication and excellent organisational skills.

Effectively triage referrals into the service/s and coordinate safe discharge from the service.

To meet the travel requirements of the post as community based. Assessing patients with in care home settings / community by completing site visits and home visits

Hold responsibility for own case load and be responsible for a defined area of the service, working without direct supervision. Supervision takes the form of regular formal training and clinical reasoning sessions, peer review, case conferences. Undertake all aspects of clinical duties as an autonomous practitioner and maintain associated records.

Assist with supervision, education and assessment of the performance of students; this would be to a graduate standard and involve working with universities to ensure the standard of practice and teaching meets the standards set by the degree level qualification
