Job summary
We have an exciting role ideally suited to an experienced clinician to work as the Acute Case Manager in the Specialist Hyper Acute Rehabilitation Pathway (SHARP) at the Queen Elizabeth Hospital Birmingham.
This post has responsibility for meeting the acute case management needs of patients with complex physical, cognitive, social & rehabilitative needs across all in patient specialties, both traumatic & non-traumatic in origin.
The post holder will be integrated into a friendly & supportive multi-professional team including Consultants in Rehabilitation Medicine, therapists & nurses who have a wealth of specialist experience in delivering high quality clinical care a & rehabilitation .
This post is ideally suited to a proactive, self-directed health or social care professional who thrive on new challenges and is a confident, effective communicator & decision maker.
The post holder will liaise and negotiate extensively with internal & external agencies at a senior level & build relationships in order to optimise patient flow & reduce delays in transfer of care.
The successful applicant will have profession specific support in addition to multi-professional support. There will also be training opportunities for this post both internally and externally.
Main duties of the job
As the Case Manager for this patient group you will use your clinical expertise and a sound understanding of different pathway options both in and outside of the trust to proactively manage and influence the specialist rehabilitation pathway. You will work with the MDT at all levels to facilitate safe and timely discharges of this complex patient group from the acute setting
The ability to work under pressure, autonomously and creatively in problem resolution and capacity management is key to the successful delivery of this post
