Job summary
An exciting opportunity for a highly motivated Nurse Leader to join the innovative and well respected Support to Care Home Service which has been established since 2017 working with Solihull Borough Care Homes. The Support to Care Home Team is part of an integrated multidisciplinary community service and works in partnership with Solihull GPs, community nurses, and local care homes.
Working with care homes and Primary Care Networks to deliver the Enhanced Support to Care Home framework, you will play a key role in empowering care homes to ensure that the residents receive optimal patient centred care.
The position will require you to be a strong, visible leader. You will be responsible for the operational management of the service; you will be working with Community Matrons, Clinical Educators, Care Home Managers, Primary Care Networks, CCG and Council.
You must be able to demonstrate excellent communication skills, teaching, management and leadership abilities. You will utilise time management and organisational skills to ensure optimal delivery of the service at all times.
Main duties of the job
Applications are invited from Registered Nurses with:
A passion for care of the older adult
Leadership experience
Excellent communication skills
The ability to build effective relationships with people across organisational boundaries
Significant recent experience in a community nursing or a care home environment
Relevant degree qualification
Car driver with full licence essential to travel across the borough of Solihull.
