Job summary

Barrow & Millom Primary Care Network are looking for an enthusiastic, self- motivated and well organized Clinical Pharmacist for an exciting opportunity to work within the Practice Network. We are a forward thinking and dynamic Primary Care Network and are seeking a like-minded individual. This is an opportunity to play a key role in establishing the function of Medicine Reviews within Barrow and Millom Primary Care Network, integrating with practices and existing local resources to the benefit of local residents.

The post-holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management.

Main duties of the job

The post-holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face and telephone structured medication reviews, manage long-term conditions, ensure systems for safe prescribing, addressing public health and social care needs of patients in the GP practices that make up the primary care network. This role is pivotal to improving the quality of patient care and operational efficiencies so requires motivation to deliver an excellent service within general practice.

Job description

Job responsibilities

Duties& Responsibilities:

  • Undertake clinical medication reviews with patients and care homes producing prescribing and monitoring guidance for network practices.

  • Undertake medication review work in the most appropriate settings, which may include GP practices, care homes, intermediate care, domiciliary visits, day centres, sheltered accommodation and community pharmacies

  • Provide general practitioners with appropriate recommendations and actions plans following medication review clinics with patients to optimise the patients treatment with medicines, and ensure appropriate follow up with patients

  • Record medication review plans, recommendations and outcomes on GP computer clinical systems and own clinical notes

  • Provide proactive leadership on medicines and prescribing systems to the PCN multidisciplinary team, patients and their carers

  • Maximise the use of technology and management systems to provide PCN level management of safety, risk and information

  • Deliver training, mentoring and guidance to other clinicians and staff on medicine issues

  • Support the network practices in improving the health of their patients through the rational and safe use of medicines.

  • Undertake responsibility for areas of QoF, Quality Contract and QIPP for the network practices and undertake clinical medication reviews to proactively manage patients with complex polypharmacy

  • Work with the general practice team to ensure the practice is compliant with CQC standards where medicines are involved

  • Provide advice for the GP management of more complex patients / polypharmacy patients

  • Report medicines related incidents, contribute to investigations and root cause analyses as well as participating in serious incident investigations and multidisciplinary case reviews

  • Computer data entry/data allocation and collation, processing and recording information in accordance with practice procedures

  • Participate in the primary care network MDT liaising with key stake-holders to ensure consistency of patient care and benefit.

  • Promote the development of those working within the PCN team. Actively participate in regular co-supervision of peers and attend regular team meetings.

  • As part of the PCN multi-disciplinary team, attend relevant MDT Network meetings, providing information and feedback.

  • Act as an interface with PCN Stakeholders including (but not limited to) acute trust, community services, mental health, community pharmacy and CCG

  • Undertake and tasks consistent with the level of the post and the scope of the role, ensuring that work is delivered in a timely and effective manner

Essential

  • Degree in pharmacy with post graduate experience in pharmacy
  • Demonstrable commitment to professional and personal development
  • Registered with the General Pharmaceutical Committee
  • Member of the Royal Pharmaceutical Society

Desirable

  • Medical Prescriber
  • Be willing to engage in funded prescribing training as per PCN needs/requirements
